What is Care Compass?
Care Compass is Sistability’s local aged care navigation service — designed to help you access government-funded support through My Aged Care, including the Commonwealth Home Support Programme (CHSP) and the Support at Home (SaH) Program (previously known as the Home Care Packages Program).
Whether you’re just starting out, need help requesting an assessment, or are already approved and unsure what’s next — we guide you every step of the way.
How It Works
Choose to pay in full or via instalments — then we’ll get your initial consultation booked in.
Book Your Care Compass Package
We meet with you (and your family, if you’d like) to go through your needs, concerns and goals - then begin the formal My Aged Care process.
Initial Consultation
You won’t have to deal with My Aged Care alone again. We’re officially appointed to speak and act on your behalf.
We Become Your Representative
We request assessments or reassessments, attend with you, and advocate clearly for the services you need.
Assessment & Referral Support
While you wait, we continue following up with My Aged Care and update your family along the way.
Progress Tracking & Family Updates
Once your outcomes are received, we explain what they mean, what you’ve been approved for, and help you take the next step with confidence.
Next Steps Planned
If you’re approved for a Support at Home Program Home Care Package, we can continue supporting you and making sure you get the services and care you deserve.
Transition to Ongoing Support
If you’ve been assigned an SaH, it’s time to choose your Aged Care Provider.
