Care Compass
Support to Help You Navigate My Aged Care
Stay independent at home with the right guidance behind you.
Care Compass is a private, fee-for-service support program developed by Sistability to help seniors and families understand and navigate the Australian aged care system.
We guide you through My Aged Care, help you apply for government-funded support, prepare you for assessments, and advocate for your needs — so you can aim to access the services required to remain safely in your own home.
Aged Care Shouldn’t Feel this Confusing
If you’ve been told to “contact My Aged Care” but don’t know what to do next, you’re not alone.
Many families find themselves:
Unsure what support is available
Confused by government letters
Waiting for assessments without understanding the process
Approved for funding, but unsure what it actually means
Trying to help ageing parents while managing their own busy lives
The system is complex. The language is unfamiliar. And the responsibility often falls on families to figure it out.
Care Compass exists to make that process clearer, calmer, and more manageable.
What Care Compass Is
Care Compass is a private navigation and advocacy service.
We are not the government. We are not an assessor. And we are not the funding decision-maker.
We are your guide.
We help you:
Register with My Aged Care
Apply for CHSP or Support At Home packages
Prepare properly for assessments
Understand what assessors are looking for
Attend assessments with you (if you choose)
Clarify approval letters and next steps
Advocate for your needs
Understand how to use your package once approved
Our role is to reduce confusion, ensure you feel prepared, and support you in aiming to access the right level of help to remain independent at home.
What Care Compass Is Not
It’s important to be clear.
Care Compass is not:
A free government service
A guarantee that funding will be approved
CHSP service coordination (please get in touch if you require help with this)
Support At Home package coordination
Residential aged care management
Financial or legal advice
Government assessors make funding decisions. We cannot promise approval.
What we can promise is guidance, preparation, advocacy and clarity throughout your journey.
Care Compass vs Support at Home Coordination
These are two very different services.
Care Compass
Navigation and advocacy before or during the application and assessment stage.
This includes:
Helping you apply
Preparing for assessments
Attending assessments
Explaining outcomes
Helping you understand your options
Support At Home Coordination
This is a separate service that only applies after you have been approved and assigned a Support At Home package.
Coordination involves:
Managing your allocated budget
Organising service providers
Monitoring service delivery
Ongoing package oversight
Care Compass helps you get on the path.
Support At Home coordination manages services after approval.
If you are approved for Support At Home, you can learn more about how we can help with coordination services here.
Our Focus Is Simple
Helping older Australians stay in their own homes.
Not residential care.
Not aged care facilities.
Not nursing homes.
Home.
Care Compass is designed to help you access government-funded support that allows you to continue living safely, comfortably, and independently where you feel most secure.
