What Is Fully Coordinated?
Fully Coordinated gives you dedicated care coordination with our Aged Care Manager, assessment and documentation support, service sourcing and engagement (including safety screening, availability , rates etc) and high needs coordination.
Here’s how it works:
Trilogy Care manages the setup, budget, and government compliance of your Support at Home (SaH) Program
Sistability coordinates your services and support team, working with you to design and deliver a care plan tailored to your life
You get more hours of care, reduced admin stress, and one-on-one support.
How Sistability Supports You
We act as your dedicated care coordinator, taking the pressure off while keeping you in control. You can choose to receive services directly from Sistability or have us coordinate trusted providers on your behalf.
Our care coordination can include:
Sourcing and scheduling services that suit your needs and personality, including allied health professionals, transport, exercise classes, etc.
Coordinating cleaning, gardening, meal support, and personal care
Managing service bookings and provider communication
Workforce compliance support (qualification checks, insurance and police checks)
Providing regular updates and advocacy for your well-being
Need extra help at home? Sistability can also deliver services directly through our in-house team of local, experienced support workers.
What Trilogy Care Does
As your approved Support at Home (SaH) provider, Trilogy Care handles:
Setting up your SaH
Budgeting and invoicing
Periodic care plan reviews
Government reporting and compliance
Payment of invoices and reimbursements
Provision of monthly statements
Clinical Support - access to their in-house team of nursing staff (phone support)
