Being assessed and approved for Support At Home funding is one step.

Once you receive your Support At Home assignment letter, you need to choose an approved aged care provider. This will activate your funding.

Home Care Packages (HCP) have now transitioned to Support At Home (SaH)

The former Home Care Packages (HCP) Program has transitioned to the new Support At Home program.

If you previously had a Home Care Package, your funding now sits under Support At Home - and you can change providers at any time.

Your approved funding level stays the same when you switch.

If you’ve received your allocation letter, you have a limited timeframe (56 days) to select your provider so your funding can be activated.

What Support At Home coordination actually involves

Support At Home coordination (previously known as care management) includes:

  • Developing and reviewing your support plan

  • Monitoring your budget and expenditure

  • Organising service providers

  • Liaising with allied health and trades

  • Troubleshooting service issues

  • Adjusting supports as needs change

  • Advocating when something isn’t working

It is proactive, ongoing oversight — not just paperwork.

Without active coordination, families often find themselves chasing providers, monitoring budgets and resolving service gaps themselves.

Services we deliver and coordinate

Depending on your approved funding level, we can both deliver services directly and coordinate additional supports through trusted providers.

Delivered In-House by Sistability

Where appropriate, we can provide services directly through our local team, including:

  • Cleaning and household assistance

  • Gardening and lawn maintenance

  • Home support services

This allows for consistent communication and direct oversight.


Coordinated Through Trusted Providers

We also coordinate services such as:

  • Personal care

  • Podiatry

  • Physiotherapy

  • Occupational therapy

  • Nursing support

  • Transport and assistive equipment

You are not left managing multiple providers.

We remain your central point of contact and oversee the coordination of all services under your Support At Home package.
