Aged Care Navigation
Care Compass is Sistability’s local aged care navigation service. Whether you’re just starting out, need help requesting an assessment, or are already approved and unsure what’s next, we guide you every step of the way.
Care Coordination
If you have Support at Home (SaH), Sistability can act as your care coordination team, helping you find the right support services, plan and schedule your supports, manage your budget, adjust services as your needs change, and handle all the admin and follow-up so you don’t have to.
Home Support Services
We personalise support to suit your needs and routine - everyday help around the house, shopping assistance, companionship, clinical support, errands, transport, help around the house, and support with routines or reminders, to anything else you may need.

Pete - Maximising His SaH Program with Sistability
Pete is an aged care client from Suffolk Park who receives support through a Stay at Home Package managed and coordinated by Sistability. Like many older Australians, Pete wanted to remain living in his own home for as long as possible - safely, comfortably, and independently.
Since coming on board with Sistability, Pete’s care has been overseen by our Aged Care Manager, Jenn, who has worked closely with him to review his goals and make the most of his available funding.

Care Plan Management & Budget Review
We help you make sense of your Support at Home (SaH) care plan by coordinating services, managing schedules, and ensuring your supports match your goals. Our team takes the stress out of organising care so you can focus on living well, not on paperwork or phone calls.
Independence Support
We work with you to understand your routines, goals, and preferences, then match you with the right support providers who can help you stay confident, active, and in control of your daily life. Whether you need gentle prompting, a supportive presence, or someone to check in when needed.
Clinical Support
When clinical needs arise, we work in partnership with qualified health professionals—such as nurses, allied health therapists, or medical practitioners—to ensure you receive safe, appropriate clinical care. This may include wound care, medication support, mobility assessments, or other health-related services to keep you well at home.
Everyday Living
Everyday tasks should feel manageable, not overwhelming. Through help with cleaning, gardening, meals, cleaning, shopping, errands, social outings, or personal routines, we make daily living easier and more comfortable so you can enjoy life at home with dignity and ease.
